We are new to the Area and are excited about the future of the home care industry.
We are passionate about giving the best care to improve patient lives and outcomes.
Duties Provide medical social services to patients in their homes in accordance with Physician orders and under the direction and supervision of the Clinical Manager or another appropriate supervisor.
Responsibilities Assist in the admission process of the patient to the agency by performing an initial evaluation, assessing the patient's psychosocial status and evaluating the patient, family and home to identify socio-economic and emotional factors that will affect plan of treatment.
Assist in the development and implementation of the interdisciplinary patient care plan as it pertains to medical social work.
Observe, record and report changes in patient's emotional and social factors that affect patient's illness and his/her need for care and response to treatment.
Consult with attending Physician concerning alteration of the plan of treatment.
Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
Evaluate the patient's and family's response to effectiveness of the medical social work intervention.
Confirm on weekly basis, the scheduling of visits with the Clinical Manager to facilitate coordination off other staff visits.
Participate in Agency activities and committees when appropriate, to include staff development activities and in-service education.
Supervise the social work assistant.
Participate in discharge planning.
Assume responsibility for self-development by continually striving to improve Medical Social Worker practice though formal education, attendance at workshops and conferences.
Active participation in professional and related organizational meetings, individual research and reading.
Skilled Medical Social Worker Services Assess the patient's potential to cope with social and health problems.
Act as consultant to the members of the health team; assist them in understanding the social, emotional and environmental factors related to the patient's health problem.
Help patients utilize the resources of their families and community.
Provide rehabilitative and supportive casework.
Job Conditions The ability to communicate well both verbally and in writing.
(English is required) The ability to access patient's homes which may not be routinely wheelchair accessible.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
Physical activities will include walking, sitting, stooping, standing and minimal to maximum lifting of patients and turning of patients.
Equipment Operation Utilization of calculator, multi-line telephone, copy machine and hand washing materials.
Qualifications Master's Degree from a school of Social Work.
Approved by the Council of Social Work Education.
Must have or in the process of acquiring a certification from the Academy of Certified Social Workers.
Two years of experience preferred.
With at least one year of experience in a healthcare setting (hospital, clinic, rehabilitation center etc.
) Must have a criminal background check.
Must have current CPR certification.
Online certification not accepted.
Must be licensed in the state of Wisconsin.
